Brigade Health is purpose built to support the needs of patients with dementia and the caregivers.  Brigade Health works with physician groups to help extend their practices beyond the traditional clinic walls to meet patients where they are and provide wrap-around services including care planning, navigation, support with transitions of care, and much more.  Brigade Health coordinates with the patient’s existing care teams or can provide direct primary care services through Brigade Health’s in-house medical group.
